How we support children/young people with SEND starting at our school

We have a variety of transition sessions for children with SEND and work closely with home to ensure we tailor a package to suit the individual child. We also work closely with feeder nurseries to ensure that we provide a smooth transition.

How we support children/young people with SEND moving between classes in our school 

We have a variety of transition sessions for children with SEND and work closely to ensure we tailor a package to suit the individual child. In our school every child will already have had interactions with many of the staff in school, either through break time duties or collective worships– this in turn supports the transition process.  Each child is seen as an individual and  we are able to cater for specific SEND needs around transition.  A SEND child depending on their needs will be provided with an ‘Social Story’ book to support a smooth transition for the family to look at before and during the summer break.

How we support children/young people with SEN leaving our school 

For every child with SEND who is moving into secondary education, or changing schools, we closely liaise with the transition staff / SENDCO and possibly the Head of Year of the next school. St Mary’s has excellent relationships with our main secondary schools and staff work closely together.  We share information with the other setting the pupil is moving to. We will agree with parents and pupils which information will be shared as part of this and complete SEND passports.

St Mary’s works very closely with our main secondary schools in the local area and children attend  transition activities, sports events, collaborative projects etc from Year 5 onwards.

All pupils have a range of induction sessions/opportunities at their new school, depending on the school they have chosen to go to. There are some occasions when extra provision is needed and this is negotiated with the Secondary school.
